As a patient, you will be required to provide demographic information and a medical history. Once your Consultation has been completed, a surgical plan will be established by Dr. Almonte, and a proposal will be prepared for you in writing. When you are ready to move forward, a deposit of 25% is required to schedule your procedure. This will secure your date of surgery. The remaining balance will be due one week prior to surgery. A cancellation/rescheduling policy will be provided for you when you schedule.
